major revision

Methods still require major revision. Administrative preregistration conditions do not determine this verdict.

Round 3 dispositions

Comment Status Assessment
C3-1 Partly resolved Preference for survival estimates, extraction of missing follow-up, and sensitivity exclusions help. They do not justify interpreting incompletely observed event proportions as hazard ratios. Reason for declining revision is insufficient: previous proposal allowed censoring-aware synthesis, not mandatory exclusion whenever censoring occurs.
C3-2 Resolved Identical component sets and thresholds now required within definition strata. Creatinine doubling separated from 50% eGFR decline; O50 follows corrected rule.
C3-3 Resolved Shorter reference arms excluded from baseline estimation. Constant-hazard interpolation within observed follow-up is explicitly acknowledged. Separate new competing-risk problem remains below.
C3-4 Partly resolved Separate pen and switching strategies now required. However, generic switching row remains an active node without specified switching threshold or algorithm.
C3-5 Partly resolved Restricted sensitivities now rated separately; matching evidence sets required. New count-derived contribution method fails for zero-event arms.
C3-6 Resolved PR04 and PR08 include O24C; narrative names both SMD outcomes.
C3-7 Resolved Recurrent events use reported log rate ratios; count models require amendment.
C3-8 Resolved O21 mapping now agrees with O21B. Other expanded composites excluded from pooling.
C3-9 Resolved Comparator activation now requires identified molecule or procedure and class, without inappropriate numeric-dose requirement.

Major findings

  1. Persistent: harms likelihood still does not handle censoring. protocol/protocol.qmd:464, “Event outcomes and harms.”

Calling counts a dichotomous outcome does not establish their subsequent hazard-ratio interpretation. Similar censoring distributions across arms are insufficient, even under proportional hazards. For example, complete-follow-up risks of 0.50 and 0.75 imply HR 2. With 20% independently censored immediately in each arm, recorded proportions become 0.40 and 0.60; their complementary log-log contrast implies HR 1.79. This example passes both proposed sensitivity cutoffs.

RoB assessment identifies uncertainty but does not repair the likelihood. Cochrane distinguishes known fixed-time event status from censored survival data. Cochrane Handbook, section 6.8

Replacement: “Use raw counts in a complementary log-log likelihood interpreted as a hazard ratio only when event status is known for all participants at a common within-trial horizon. Otherwise use reported or reconstructed survival contrasts or a likelihood accounting for censoring. Counts with unresolved incomplete observation may enter a separate sensitivity analysis of recorded harms, without hazard-ratio interpretation or fixed-horizon absolute-risk conversion.”

This preserves usable harms evidence while correcting the estimand.

  2. New: contribution calculation is undefined for eligible sparse networks. protocol/protocol.qmd:823, “Confidence in cumulative evidence.”

Count-derived log hazard contrasts require differences of log(-log(1 − r/n)). This transformation is nonfinite when an arm has zero events or all participants experience events. No boundary correction, variance specification, or fallback is supplied.

Consider two trials: A versus B with 0/100 versus 10/100 events, and B versus C with 10/100 versus 0/100. This connected three-node network meets the 20-event rule, yet neither trial supplies a finite uncorrected contrast for the proposed inverse-variance contribution network. Dropping trials would recreate C3-5.

Replacement: “Auxiliary inverse-variance contribution matrices are used only when valid finite contrasts and standard errors are available for every contributing trial, with multi-arm covariance preserved. When this condition fails, within-study bias and indirectness are judged qualitatively across the full primary evidence set, with supporting studies and reasons recorded. A reduced auxiliary network must not be represented as the primary network’s contribution matrix.”

Any alternative numerical method needs an explicit zero-cell rule and justification before registration.

  3. New: revised baseline-risk rule treats incompatible survival quantities as interchangeable. protocol/protocol.qmd:767, “Absolute effects.”

“Kaplan-Meier or cumulative incidence” does not specify how competing events are handled. For outcomes such as kidney failure or heart-failure hospitalization, death can preclude the event. One minus Kaplan-Meier survival with competing deaths censored differs from cumulative incidence accounting for those deaths. Cause-specific HRs and subdistribution HRs also cannot share an unspecified conversion rule. Competing-risks methods

Replacement: “Extract whether reported hazard ratios are cause-specific or subdistribution estimates and synthesize them separately. For outcomes with competing events, estimate absolute event probabilities using cumulative incidence. Convert cause-specific hazard ratios only through a model including both event and competing-event hazards; when required information is unavailable, report relative effects without absolute conversion. Subdistribution hazard ratios use a matching cumulative-incidence baseline. Kaplan-Meier complements must not substitute for competing-risk cumulative incidence.”

Minor finding

  1. New: switching-strategy row lacks template protection. protocol/nodes.csv:97, IGLARLIXI_SWITCH_SC_TIT_QD; protocol/protocol.qmd:552, template activation rules.

is_template = no exposes a generic switching label as an eligible node despite missing strategy values. Narrative separation rule is appropriate, but table should enforce it.

    Replacement values: node_id = IGLARLIXI_SWITCH_<strategy>_SC_TIT_QD; is_template = yes.

Replacement note: “Excluded from networks until a unique strategy identifier, permitted pens, switching threshold, and titration algorithm are specified. Each distinct strategy receives its own instantiated node.”

Add this third template category beside dose and comparator templates.
