Round 2 comments

G2-1. Resolved. O05 is off W06 and W07. P01 uses W01 (primary, target 52, limits 36 to 64), W02 (secondary, target 26, limits 20 to 36), and W03 (secondary, target 104, limits 64 to 130). P02 uses W27 (primary, target 68, limits 40 to 104) and W28 (secondary, target 26, limits 20 to 40). After matching each window to the populations that outcome actually includes, no outcome and population pair has overlapping windows or two primary windows.

G2-2. Resolved. Reformulated 1.5 mg is paired only with original 3 mg (SEMA_PO_3MG_QD), 4 mg only with 7 mg (SEMA_PO_7MG_QD), and 9 mg only with 14 mg (SEMA_PO_14MG_QD). The networks section states those three pairs only.

G2-3. Resolved. Population specific horizon rows were declined in favor of capping every hazard horizon at the median follow-up of that network's reference arms. That reason is sound. It keeps a stored 156 week value from being applied in a heart failure network of about one year, and it stops extrapolation in other short networks, while 156 weeks remains available when reference arms are followed that long. The stored field is the uncapped horizon. The cap is the analysis rule.

G2-4. Resolved. rare_outcome is set before extraction and is never recomputed from observed risks. Keeping O35A, O36B, and O120 off that flag is sound: O35A uses an absolute 5 per 1,000 at 52 weeks, O36B uses 50 per 1,000 procedures, and O120 uses 5 per 1,000 pregnancies. PR06 and the ratio range stay on the flagged rows.

G2-5. Resolved. W00 names its outcomes, including O04 and O04B. W25 is longest_follow_up for O51 (lower 52, upper blank). Blank upper means no upper limit, and nearest_target does not apply to longest_follow_up.

G2-6. Resolved. The P01 insulin stratum is one field, with a comma between prandial and premixed.

G2-7. Resolved. EM02 includes P03 and P04, and the unit is BMI standard deviation score in those populations.

G2-8. Resolved. O21B includes P07A and P07B and is never pooled with O21. O22B includes P08A and P08B and is never pooled with O22. O22C also covers P08A and P08B. O50 remains the 50 percent kidney composite for P08A and P08B.

G2-9. Resolved. Each harms_nrs.csv row repeats exposure, comparators, and design. Initial treatment and on treatment are pooled separately and never combined, matching the synthesis section.

G2-10. Resolved. Rare outcomes use the ratio range 1/1.25 to 1.25. Incoherence on a ratio scale uses the difference of log effects against plus or minus log 1.25.

G2-11. Resolved. O120, O36A, and O36B have blank absolute_horizon_weeks, and absolute effects are per 1,000 procedures or pregnancies. A window mismatch for these outcomes is listed under new issues.

G2-12. Resolved. If placebo and standard care are unconnected, the reference node is the active node that appears in the most trials.

G2-13. Resolved. SEMA_PO_FLEX_QD has formulation "original tablet".

G2-14. Resolved. Rows with is_template = yes stay out of every network until a numeric dose replaces a dose placeholder. The named comparator template is listed under new issues.

G2-15. Resolved. Within study bias, reporting bias, and indirectness are taken once from the frequentist contribution matrix and apply to both frameworks. Imprecision, heterogeneity, and incoherence are judged on each framework's estimates, and the lower rating is reported. Inestimable incoherence is recorded and judged from transitivity and network structure, with the reason stated.

New issues

Major

O24C has no relative effect prior and no heterogeneity prior (outcomes.csv O24C; priors.csv PR04 and PR08; protocol.qmd, Effect measures). O24C is an SMD for total HRQoL, populations All, threshold 0.2 SMD, never pooled with O24B. W24 is its primary window. reference_sd.csv sets s_y = 1 for O24B and O24C, and PR01 covers continuous baselines. PR04 (relative effect d) and PR08 (tau) apply only to O24B. Effect measures says "standardized mean differences only for O24B," which contradicts the outcome table. No row states a prior for O24C, so the Bayesian primary model for that outcome cannot be run.

Replacement. Effect measures: "standardized mean differences for O24B (physical functioning) and O24C (total HRQoL), each in its own network." priors.csv: set applies_to on PR04 and PR08 to O24B;O24C, keeping Normal(0, 2.5^2) for d and Half-normal(0, 0.5^2) for tau. Keep s_y = 1.

Minor

1. Named comparator template (nodes.csv, OTHER_ACTIVE_<name>; protocol.qmd, Networks and nodes). The row is a template, the placeholder is <name>, and the dose is blank, while the note and the protocol sentence say a numeric dose replaces <dose>. Comparators are molecule level nodes. Replacement note: "Template, not a node: excluded from every network until the comparator name replaces <name>. Doses of that comparator are not separate nodes." Limit the numeric dose sentence to dose templates.

2. Window rule for procedure and pregnancy outcomes (protocol.qmd, Time windows; windows.csv W00; outcomes.csv O36A, O36B, O120). The text says every outcome and population has exactly one primary window. O120 is on no window. O36A and O36B are risks per procedure and still sit on W00, which selects the longest follow-up in weeks. Replacement: "Per procedure outcomes (O36A, O36B) and the per pregnancy outcome (O120) have no week window." Remove O36A and O36B from W00.

3. Recurrent event likelihood (protocol.qmd, Bayesian network meta-analysis). The likelihood list still offers a negative binomial for recurrent events. O04B, the event outcomes paragraph, and PR14 allow only reported log rate ratios, and a count model requires an amendment. Replacement: "O04B uses a normal likelihood on reported log rate ratios. A negative binomial model requires a logged amendment."

4. Relative threshold omits O04B (protocol.qmd, Priors, Absolute effects, and CINeMA imprecision; outcomes.csv O04B). Those sentences attach the ratio range to rare_outcome = yes. O04B is rare_outcome = no, with a relative threshold of 1.25 (rate ratio) and prior PR14. Replacement: "The same relative range applies to O04B (rate ratio 1.25), which uses PR14." State that range in the CINeMA imprecision bullet as well.

5. Stored horizon versus the cap (outcomes.csv, absolute_horizon_weeks; protocol.qmd, Absolute effects). Several hazard rows still store 156 weeks, including O20, O21, and O23, which include P07A and P07B. A reader who lets the table override the prose can restore 156 weeks in a short network. Replacement: "absolute_horizon_weeks is the uncapped horizon. The analyzed horizon is the minimum of that value and the median follow-up of the network's reference arms."

6. O22C mixes eGFR thresholds (outcomes.csv O22C). The definition is a kidney composite "as in O22 or O22B" plus cardiovascular death, so a 40 percent decline plus cardiovascular death can share an outcome id with a 50 percent decline plus cardiovascular death. The response adopted this wording. Replacement: give the 40 percent plus cardiovascular death composite its own outcome id, and map a trial to that id unless it also reports the 50 percent version.

7. Percent weight label (protocol.qmd, Time windows; windows.csv W09). The prose assigns the 12 to 52 week window in P12 and P15 to O10. Those populations use O10B. W09 is primary for O10B, target 24, limits 12 to 52. Replacement: "P12 and P15 use O10B (W09, target 24, limits 12 to 52). Other populations use O10 (W06, target 68, limits 40 to 104)."

The following checks held: every log odds, log hazard, cloglog, and log rate outcome is in turner_map.csv; every mean difference and standardized mean difference has a reference SD; nrs_synthesis = yes matches harms_nrs.csv; node and agent class ids exist; the 43 agents and their search terms match concepts.yaml 1.2.0; HRS9531 and HRS-7535 stay separate. The odds ratio sensitivity limited to 20 to 104 weeks, and GRADE for nonrandomized harms starting at high certainty and rating down with ROBINS-I, are coherent.

Pre-registration conditions

Complete these owner supplied items before registration: team names and affiliations; funding; competing interests; confirmation of Embase access, with the omission reported as a limitation if access is not available; PRESS 2015 by a human information specialist; verification of the unverified Chinese candidate names by a Chinese reading team member; locking of provisional thresholds by clinical advisors and patient partners. The timeline already requires a reconciled harvest of the registered concepts version on all four API sources before the baseline search. The 2026-10-12 run remains a pipeline test on concepts 1.0.0.

Judgment

All fifteen round 2 comments are resolved, including the horizon cap and the decision to leave O35A, O36B, and O120 on absolute thresholds. One prespecified outcome, O24C, still has no relative effect prior and no heterogeneity prior, and the effect measures sentence contradicts its row. The other new items are local sentence or row fixes. With the O24C priors added, those fixes made, and the administrative conditions completed, the methods are acceptable for registration.
