major revision

Methods still require major revision. Administrative pre-registration conditions do not drive this verdict.

Round 2 dispositions

Comment Status Assessment
C2-1 Partly resolved Procedure outcomes corrected. Proportional-hazards argument is valid with complete ascertainment at common follow-up; similar arm follow-up does not address censoring.
C2-2 Resolved Odds-ratio sensitivity distinguished from primary hazard analysis; inverse-variance eligibility now requires events and non-events.
C2-3 Partly resolved MACE, physical-function and abstinence definitions improved. Kidney composites still combine different component sets and thresholds.
C2-4 Resolved Outcome/population intersections eliminate overlapping windows. O04B selection, narrow-window limits and weight units specified.
C2-5 Partly resolved Fixed-time horizons repaired. Median follow-up cap does not prevent extrapolation from shorter reference arms.
C2-6 Resolved Observational syntheses separate populations, contrasts, estimands, latency, risk windows, measures and designs; minimum adjustment enforced.
C2-7 Partly resolved Flexible-dose definitions and numeric ratios improved. iGlarLixi still combines distinct formulations and switching strategies without justification.
C2-8 Resolved Direct-minus-indirect interval rule covers previously unclassified cases; non-estimable incoherence addressed. Separate new contribution-matrix problem remains.
C2-9 Partly resolved SMD reference scale and O04B prior supplied. Narrative still permits negative-binomial model expressly excluded by authoritative table.
C2-10 Resolved Single amycretin entry retains zenagamtide and development-code aliases in agent and search tables.
C2-11 Resolved EM02 populations corrected, heart-failure pooling prohibited, categorical coding and class-specific identifiability specified.

Major issues

  1. Persistent: censoring remains unaddressed in harms likelihood. protocol/protocol.qmd:451, “Event outcomes and harms.”

Response correctly distinguishes proportional hazards from constant baseline hazards. I accept that mathematical clarification. However, participants with censored follow-up cannot automatically contribute as non-events at trial end. Equal follow-up distributions across arms do not repair this. Cochrane Handbook, section 6.8 requires known fixed-time status for dichotomous analysis.

Replace with: “Count-based complementary log-log synthesis requires complete event ascertainment at a common time within each trial. With censoring, use reported or reconstructed survival estimates or a likelihood accounting for censoring. Without adequate survival information, report descriptively rather than treating censored participants as non-events.”

  2. Persistent and new: kidney outcome definitions remain incompatible. protocol/outcomes.csv:23, O22, O22C, O22D and O50.

O22C still combines 40% and 50% eGFR thresholds. New O22D combines different albuminuria and eGFR component sets. Separate broad labels do not establish equivalent endpoints.

New mapping of creatinine doubling to 50% eGFR decline is also incorrect. Conventional CKD-EPI mapping is approximately 57%. Primary analysis

Replace mapping rules with: “Synthesize separately by component set and eGFR threshold, assigning distinct outcome identifiers or definition strata. Retain creatinine doubling as a distinct threshold; do not equate it with 50% eGFR decline.” Apply corresponding correction to O50.

  3. Persistent: horizon cap does not enforce stated extrapolation restriction. protocol/protocol.qmd:738, “Absolute effects.”

Reference-arm follow-ups of 13, 26 and 52 weeks produce a 26-week cap, beyond first arm’s observed follow-up. Pooling their raw risks also does not produce reference risk at 26 weeks. Median cap is reasonable for selecting a reporting horizon, but insufficient justification for claimed absence of extrapolation.

Replace with: “Estimate reference survival at the selected horizon using censoring-aware estimates from arms with follow-up covering that horizon. Pool risks standardized to that horizon. Exclude shorter arms from baseline-risk estimation unless a separate extrapolation model is prespecified, justified and reported.”

  4. Persistent: iGlarLixi primary node remains underdefined. protocol/nodes.csv:95, IGLARLIXI_SC_TIT_QD.

Row explicitly pools single-pen regimens with switching regimens. “Pen switching follows the trial algorithm” does not define one common strategy. Response supplies no reason for retaining this pooling despite accepting formulation and algorithm distinctions.

Replace with: “Define separate primary nodes for 100 U/50 micrograms per mL, 100 U/33 micrograms per mL, and each switching strategy. Switching nodes specify permitted formulations, switching thresholds and titration algorithm. Combine strategies only in a prespecified sensitivity analysis.”

  5. New: certainty ratings borrow contributions from different evidence. protocol/protocol.qmd:786, “Confidence in cumulative evidence,” versus “Frequentist network meta-analysis.”

Restricted odds-ratio sensitivity may omit trials contributing to primary hazard analysis. Its contribution matrix therefore assigns no contribution to omitted trials when rating primary evidence. Connectivity may also differ. Automatically retaining lower certainty can conflate differences in evidence with differences between statistical frameworks.

Replace with: “Contribution assessments must represent the studies and network supporting each estimate. Share contribution-based judgments only when evidence sets match and contribution assessments are applicable. Rate primary hazard analyses separately from restricted odds-ratio sensitivities; report sensitivity findings without automatically substituting their certainty rating.”

Minor issues

  1. New: O24C lacks relative-effect and heterogeneity priors. protocol/priors.csv:5, PR04 and PR08; protocol/protocol.qmd:476, “Effect measures.”

Change both applies_to cells from O24B to O24B;O24C. Replace “standardized mean differences only for O24B” with “standardized mean differences for O24B and O24C.”

  2. Persistent contradiction after partial repair: recurrent-event likelihood. protocol/protocol.qmd:568, “Bayesian network meta-analysis.”

Negative-binomial option contradicts O04B and PR14, which require an amendment before fitting count models. Replace with: “Normal likelihood on reported log rate ratios with valid standard errors for recurrent events.”

  3. New inconsistency: obsolete expanded-MACE mapping. protocol/outcomes.csv:21, O21 versus O21B.

O21 still assigns composites adding revascularization to O21B, whose revised definition excludes them. Replace O21 mapping with: “Three-point MACE plus hospitalization for unstable angina maps to O21B. Other expanded composites are described and not pooled.”

  4. New: comparator template cannot satisfy activation rule. protocol/nodes.csv:133, OTHER_ACTIVE_<name>; protocol/protocol.qmd:529.

Template requires replacing <dose>, but contains <name> and represents a molecule-level comparator.

Replace with: “Intervention dose templates require a numeric randomized maintenance dose. Comparator templates require an identified molecule or procedure and assigned class; dose is not required for molecule-level comparator nodes.”
