major revision

Methods still require revision. Administrative checklist items do not drive this verdict.

Major findings in version 0.2.0

  1. Harms model assumes more than stated. protocol.qmd, “Event outcomes and harms” requires only constant hazard ratios. Its follow-up-offset interpretation requires constant arm-specific hazards and adequate handling of censoring. NICE explicitly requires stronger assumptions. Replace: “Use reported survival estimates where available. Use count-based cloglog models only when constant hazards and follow-up assumptions are justified; otherwise analyze risks within compatible time windows.” For outcomes.csv, O36A/O36B, replace log_hazard_cloglog with log_odds, remove 52-week horizons, and define risks per procedure. NICE TSD2, section 3.2

  2. Frequentist fallback changes effect measure. protocol.qmd, “Frequentist network meta-analysis” applies Mantel-Haenszel to all binary outcomes, including those assigned hazard ratios. netmetabin does not estimate HRs. Also, requiring events in every arm misses zero non-events. Replace: “Use inverse variance log-OR analysis only when every arm contains events and non-events. MH analyses are common-effect OR sensitivities for compatible follow-up windows, not checks of the identical HR model. Report contributing studies and connectivity separately.” Package documentation

  3. New outcome subdivisions still permit incompatible pooling. In protocol/outcomes.csv, O22B combines different eGFR thresholds and cardiovascular-death inclusion; O21B combines different additional cardiovascular components; O24B combines total HRQoL with physical function. Replace mapping rules: “Create separate syntheses for each component definition; pool SMDs only within the same construct.” Add P08A/P08B to appropriate broader kidney outcomes. For O110B/O111, replace unspecified abstinence periods with separate duration and verification strata; never combine continuous abstinence with point-prevalence abstinence.

  4. Time-window assignment is contradictory. protocol/windows.csv, W01/W06 both assign primary windows to P01/O05; W02/W07 and W03/W06 also overlap. Remove O05 from W06/W07 and create equivalent O05-only rows restricted to P02. Add: “Window mappings apply only where outcome population eligibility also matches.” Specify O04B follow-up selection and numeric limits for the promised “narrow windows” sensitivity. Update protocol.qmd, “Time windows,” to distinguish kilogram weight from percent weight.

  5. Absolute horizons cannot be obtained from arbitrary follow-up. protocol.qmd, “Absolute effects” assigns fixed horizons even to secondary windows. For example, O11 analyses at 26 weeks retain a 68-week horizon. Replace: “For fixed-time outcomes, use the analyzed window’s target and reference risks from that window. Do not extrapolate odds ratios across horizons. For survival outcomes, use reference survival at the specified horizon; publish and justify any extrapolation model.”

  6. Observational estimands remain insufficiently separated. protocol/harms_nrs.csv, O28 and inherited “As above” rows, allow initiation and on-treatment effects, while protocol.qmd:701 stratifies only by measure, design and comparator type. Replace: “Pool separately by population, exposure/comparator contrast, intercurrent-event strategy, latency and risk window, effect measure, and design. Require the listed minimum confounder adjustment or explain exclusion from quantitative synthesis.” This prevents incompatible adjusted HRs from entering one pool.

  7. Flexible-node rules still collapse different randomized regimens. protocol.qmd, “Networks and nodes,” defines flexible nodes only by molecule, route and frequency. Replace: “Flexible nodes additionally encode formulation, permitted maintenance-dose set and dosing algorithm.” In protocol/nodes.csv, replace ICOSEMA’s “fixed ratio as marketed” with “350 U insulin icodec per 1 mg semaglutide.” Specify ratio-specific or explicitly defined pen-switching strategies for IGLARLIXI rather than “33 or 50.” COMBINE 2

  8. Certainty algorithm leaves results unclassified. protocol.qmd, “Confidence in cumulative evidence”: with threshold (T), direct effect (2T) and indirect effect (3.5T) meet none of the incoherence rules. Networks without identifiable inconsistency parameters cannot receive design-by-treatment judgments. Replace: “Assess the direct-minus-indirect contrast and its interval against ±T: no concerns if entirely within, major concerns if entirely outside, otherwise some concerns. When incoherence is not estimable, record that limitation and justify the judgment from transitivity and network structure.”

Minor findings

  1. Prior specification remains incomplete. protocol/priors.csv, PR01, lacks s_y for SMDs; PR05/PR06 require a reference risk unavailable for O04B; negative-binomial dispersion has no prior. Concrete minimal repair: set O24B s_y=1; assign O04B Normal(0,2.5²) relative effects; restrict O04B synthesis to reported log-rate ratios with valid SEs until a complete count-model specification is prespecified.

  2. One drug appears twice. protocol/agents.csv, rows amycretin and zenagamtide, and corresponding concepts.yaml keys, represent the same molecule. Merge under one canonical identifier, retain both search names, assign AMY_GLP1, and map existing AMYC nodes to that identifier. Manufacturer confirmation

  3. Table/text reconciliation remains necessary. Add P03 to protocol/effect_modifiers.csv, EM02. Remove permission to pool P07A/P07B from protocol.qmd, “Participants,” to match their authoritative “Never pooled” rows. In “Network meta-regression,” replace universal centering/scaling with: “Standardize continuous covariates; use prespecified indicator coding and reference categories for categorical covariates. Require identifiable class-specific coefficients with adequate within-class information.”

Disposition of all round 1 comments

Comment Status Reason
C1 Partly resolved Intransitivity prohibition and phenotype separation fixed; HF sensitivity contradicts authoritative rows.
C2 Partly resolved BMI and cognition split correctly; O24B still mixes constructs.
C3 Partly resolved Directions and substances fixed; abstinence duration and verification remain insufficiently specified.
C4 Partly resolved Several splits work; broader composites still mix definitions.
C5 Partly resolved Survival hierarchy improved. Blanket harms exception lacks required assumptions; NICE citation does not justify it.
C6 Partly resolved Common-effect labeling is appropriate; scale mismatch and zero non-events remain.
C7 Resolved Intercurrent-event strategies separated; primary substitution prohibited.
C8 Resolved Cluster corrections, ICC sensitivities and crossover fallback now executable.
C9 Partly resolved Eligibility and target specifications added; pooling still needs estimand and latency separation.
C10 Resolved ROB-MEN and ROB-ME assigned correctly.
C11 Partly resolved Shorter obesity trials included; overlapping windows and unrestricted harms assumptions remain.
C12 Partly resolved Reference node and horizons declared; horizon-compatible risk estimation remains incomplete.
C13 Partly resolved Wider log-effect prior has defensible responder-effect rationale, subject to calibration; nuisance specifications remain incomplete.
C14 Partly resolved Strategy fields added; flexible dose sets and component ratios remain ambiguous.
C15 Resolved Withdrawal eligibility no longer requires weight-loss response; run-in modifier added.
C16 Resolved S3/S4, assignment threshold and replacement-regimen rules supply operational classifications.
C17 Resolved Any comparator eligible. Restricting comparator-only trials is defensible given explicit scope and acknowledged connectivity costs.
C18 Resolved CENTRAL committed, core queries reproducible, independent benchmarks planned. Owner-supplied conditions remain exempt.
C19 Resolved Explicit therapeutic-drug scope justifies excluding physiological native-infusion experiments; historical aliases added.
C20 Resolved Named doses added. Extraction-based reconciliation is reasonable; registry candidates appropriately remain aids.
C21 Resolved HRS9531 and HRS-7535 separated correctly.
C22 Partly resolved Priority, missingness and overlap improved; categorical coding and class-specific identifiability remain incomplete.
C23 Resolved Administrative conditions tracked; absent prospective protocols now labeled correctly.
