Peer review round 17: LivingGLP1 protocol 0.17.0 (reviewer grok-4.7)

1. Round 16 comments

G16-1. Resolved. Absolute effects now gives each outcome class one one arm procedure. For an outcome without a competing event, a single usable reference arm is analyzed with its own likelihood and prior PR02, and between arm heterogeneity is not estimated. For an outcome with competing events, that section returns the case to Competing risks: (\tau_k) is not estimated, (u_{\mathrm{new}}) is not drawn, and each draw of the reference log hazards is from (\mathrm{N}(y_a, V_a)).

For an outcome without a competing event, a usable arm is one whose follow-up covers the analyzed horizon and which supplies a qualifying input. An arm that lacks a qualifying input is listed with the reason and left out of the reference risk and the scenarios. Two arms that merely cover the horizon, only one of them usable, no longer produce a heterogeneity variance.

G16-2. Resolved. The count inputs are ordered. A Kaplan-Meier cumulative incidence at the horizon is used when (0 < F < 1), with the standard error specified below. Otherwise, an event count reported with valid person time over one period enters the Poisson likelihood with mean (e^{\theta_a} E_a / t). Otherwise, the participants with an event enter the binomial complementary log log likelihood. An arm that reports both count inputs enters only the Poisson likelihood, so the arm contributes one count likelihood. Valid person time remains the definition in Competing risks. (f_a) is the longest follow-up at which status is known for every randomized participant, and that time must reach the analyzed horizon. The 95 percent rule remains the sensitivity analysis in Event outcomes and harms.

G16-3. Resolved. The PR02 rationale now states three uses of (\mathrm{Normal}(0, 5^2)), with sensitivity (\mathrm{Normal}(0, 10^2)). In the network meta-analysis, the complementary log log offset is log follow-up in years, so the trial baseline is a log cumulative hazard per year, meaning (\log(\Lambda(t)/t)). In the reference arm meta-analysis of a hazard based outcome without a competing event, the same prior is placed on the mean log cumulative hazard at the analyzed horizon, (\log \Lambda(t)). Fixed time reference arm models use the logit scale. Restricting the per year description to the network model is the right split, and the logit clause matches the binomial logit likelihood already specified for fixed time outcomes. The prior values are unchanged.

2. New issues

The revision introduces no new error, inconsistency, or non-executable rule.

The scenario risk (1 - \exp(-e^{\theta_a})) is the cumulative incidence at the horizon when (\theta_a = \log \Lambda_a(t)). With (\theta_a = \log(\lambda t)), the Poisson mean (e^{\theta_a} E_a / t) equals (\lambda E_a), and the binomial offset (\log(f_a / t)) places that same (\theta_a) on the complementary log log scale. The map from (\theta_a) to risk is monotone, so for one usable arm the risk at the posterior median of (\theta_a) is the median of the predictive distribution of the risk. With two or more usable arms, the displayed reference risk remains the median of the random effects predictive distribution, and the scenarios are the 25th and 75th percentiles of the arm risks under PR02 alone, redrawn independently.

In Competing risks, boundaries at or before the analyzed horizon must match across covering arms, and a boundary after the horizon counts as the horizon. The working normal likelihood and the zero count restriction on those routes are unchanged. Count likelihoods for those routes stay with the Aalen-Johansen horizon item on the pre-analysis amendment. The routes as written can be carried out.

3. Checks and registration

Against the version 0.16.0 snapshot, the authoritative edits are in protocol.qmd (the version lines, the piecewise boundary sentence, the reason for the working normal likelihood, and the absolute effects passage) and in priors.csv (the PR02 rationale). amendments.csv adds A016, dated 2026-10-13, version 0.17.0, covering competing risks, absolute effects, and priors, citing 11 comments, before results. The other authoritative tables are unchanged. outcomes.csv has 77 outcomes. The subtitle, the status sentence, and the outcomes section identify version 0.17.0, dated 2026-10-13, after sixteen rounds. The 11 comments named in A016 are the 11 round 16 rows in comments.csv.

search/strategies/concepts.yaml remains version 1.2.0. R/search_strategy.R builds the PubMed, Europe PMC (excluding patents), OpenAlex, and ClinicalTrials.gov queries from the class terms, the agent terms, the mechanism block, and the PubMed vocabulary. The Chinese terms remain only in concepts.yaml. The renderer was read and was not executed. The Quarto file was read and was not rendered. The iGlarLixi switching row remains a template (is_template yes).

Team names, affiliations, funding, competing interests, confirmation of Embase access, PRESS review by a human information specialist, verification of the Chinese terms by a Chinese-reading team member, and confirmation and locking of provisional thresholds by clinical advisors and patient partners remain administrative pre-registration conditions. The protocol checklist also lists an OpenAlex API key and a logged harvest of all four API sources before the baseline search. Absence of those administrative items does not change this verdict. I would approve the methods for registration once those conditions are completed.
