Peer review round 17: LivingGLP1 protocol 0.17.0 (reviewer fable-5.1)
What I verified against the 0.16.0 snapshot: the revision changes protocol.qmd (subtitle; the round and version counts at lines 43 and 239; the boundary clause at lines 589 to 591; the reason for the working normal likelihood at lines 623 to 625; the scenario parenthetical at lines 1006 to 1007; the absolute effects passage at lines 1028 to 1053 and 1063 to 1066), the rationale of PR02 in priors.csv, amendments.csv (A016, whose count of 11 comments matches the eleven new rows of comments.csv; verdicts.csv carries the three round 16 verdicts) and the README status line. The eleven other authoritative tables, concepts.yaml (1.2.0) and R/search_strategy.R are byte identical to the reviewed snapshot, and protocol.html carries the 0.17.0 subtitle. Numerical checks: under a constant hazard the identity log{−log(1 − p)} = θ_a + log(f_a/t) holds exactly; the transformed limit standard error is 0.120 for the interval 0.08 to 0.125 and 0.641 for 0.005 to 0.06, as in my round 16 review; a Kaplan-Meier arm with F = 0.10 and standard error 0.12 gives a scenario risk of 0.1001 under PR02 alone, so the shrinkage is negligible; a zero count Poisson arm at a two year horizon gives a finite posterior median risk of 0.00044 with 500 person years and 0.0026 with 50.
1. Round 16 comments
| Item | Status | Reason |
|---|---|---|
| F16-1 | Resolved | Lines 1032 to 1041: Poisson when the count comes with valid person time over one period, otherwise the binomial with f_a the longest follow-up at which status is known for every randomized participant, reaching the horizon; an arm reporting both enters only the Poisson. Precedence and the known status rule are closed; "one period" and "every randomized participant" leave two residuals (items 1 and 3). |
| F16-2 | Resolved | The response says "your text adopted", but lines 1048 to 1053 drop the branch I proposed for Kaplan-Meier arms (their own value) and route every usable arm through 1 − exp(−e^{θ_a}) from its likelihood under PR02 alone, while calling it my text. On its merits the simplification is harmless (check above) and buys one rule for all arms, so I accept it; the response should have recorded a partial adoption. The zero count property is item 5. |
| F16-3 | Resolved | Lines 1028 to 1031 adopt the transformed limits verbatim. |
| F16-4 | Resolved | Lines 589 to 591 adopt the clause verbatim; with lines 592 to 593 and the single interval case at lines 661 to 662, the piecewise and exponential routes now agree on which arms match. |
| F16-note | Partly resolved | Lines 623 to 625 now state the actual choice without the thin justification, and the PR02 rationale distinguishes the per year baseline of the network model from the cumulative hazard at the horizon and the logit scale. Count likelihoods for the competing risk routes are deferred to the pre-analysis amendment; the reason (the C16-0 correction: the exact count likelihoods reach only O20 and fixed time outcomes, and the competing risk routes need a likelihood that pairs with curve arms) is sound, and I called the matter amendment level in round 16. |
2. New items in 0.17.0
No major items. None touches a relative effect model, an estimand or a relative estimate; all sit in the reference arm model of @sec-absolute.
Minor
Poisson input "over one period" (lines 1033 to 1035). The exponential approximation sums finer count intervals (lines 666 to 667); here an arm that reports events and person time on several intervals, read strictly, has no "valid person time over one period" and is pushed to the binomial or, when status is not known for everyone, to no input, although under the stated constant hazard the sums are the same input. Replace "over one period;" with "over one period, or summed over reported finer intervals;".
Reference risk with one usable arm (lines 1046 to 1047) and the
applies_tofield of PR02. "Its likelihood is used with prior PR02 and no between arm heterogeneity is estimated" does not name the reference risk, and "the median of its predictive distribution" (line 1005) has no referent with one arm. Replace with "with one, its likelihood is used with prior PR02, no between arm heterogeneity is estimated, and the reference risk is the posterior median of 1 − exp(−e^{θ_a})". Since PR02 now also sits on a single arm's θ_a (here and in the scenario rule), append toapplies_toof PR02 inpriors.csv: "; a single usable arm's θ_a, and each usable arm's θ_a for the scenario risks of @sec-absolute".Scope of "every randomized participant" for f_a (lines 1037 to 1038). The primary rule of @sec-outcomes (lines 505 to 507) reads trial wide, and only its sensitivity version says "in every arm" (lines 509 to 510). In a reference arm model only that arm's status matters; as written, losses in a treated arm disqualify a reference arm with complete status. Replace "status is known for every randomized participant" with "status is known for every participant randomized to that arm".
Scenario inputs for fixed time outcomes (lines 1005 to 1011). The parenthetical now defines the scenario inputs and their uncertainty for both hazard based branches but leaves fixed time outcomes as the bare phrase "trial reference risks": whether each arm's value is its observed proportion or its posterior under PR02, and whether scenario uncertainty is shown, is unstated. This predates 0.17.0; the revision makes the asymmetry visible. Insert before "for hazard based outcomes without a competing event": "for fixed time outcomes, each reference arm's posterior median risk under its binomial likelihood with PR02 alone, with scenario uncertainty from independent draws across arms as for hazard based outcomes;" (the label of item 5 applies).
Zero count arms under the adopted scenario rule (lines 1048 to 1053). This is a property of the rule I proposed, not an error: with zero events the likelihood only bounds θ_a from above, so the posterior median is set by the lower tail of PR02 (0.00044 at 500 person years and 0.0026 at 50 for a two year horizon) and moves further down under the sensitivity prior Normal(0; 10²). It matters only when a scenario percentile lands on such an arm, which can happen with two or three usable arms. Append after "both scenarios are its risk": "; a scenario value that falls on a zero count arm is labeled as prior dependent and is also shown under the sensitivity prior of PR02".
Editorial. (a) "Relative only rule" is used at lines 690, 699, 1047, 1057, 1060 and 1148, but the defining passage at lines 1077 to 1084 does not carry the name; insert "(the relative only rule)" after "When a valid absolute conversion is unavailable". (b) "Divided by 1.96" (line 1030) assumes a 95 percent interval; replace with "divided by the normal quantile of the reported level (1.96 for 95 percent)". (c) The matters deferred to the pre-analysis amendment (F12-note, F16-note) are recorded only in
comments.csvand the responses; append to the Amendments paragraph (lines 83 to 84): "Matters deferred to an amendment before the first analysis are those markeddeferred to amendmentinprotocol/peer-review/comments.csv."
3. Judgment on revisions made for the other reviewers
C16-0: the correction is right and the text is consistent with it. The exact count likelihoods sit inside the branch for outcomes without a competing event (lines 1032 to 1042), the fixed time branch keeps its logit binomial (line 1002), and the zero count restrictions of the competing risk routes at lines 620 to 621 and 671 to 672 are untouched. C16-1 is resolved by the usable arm definition and the two, one and none cases (lines 1042 to 1048); item 3 is the residual. C16-2 and G16-3 are resolved by the PR02 rationale. G16-1 is resolved by lines 1063 to 1066, which now restate lines 644 to 646 exactly. G16-2 is resolved by the precedence sentence at lines 1040 to 1041; item 1 is the residual.
Conclusion
All four of my round 16 items are adopted, one with an unrecorded simplification that I accept, and the note is deferred for a sound reason. The six items above are clauses inside the reference arm model with one sentence closures; none changes an estimand, a relative effect model or a registration level decision, so by the standard of rounds 12 to 16 they are amendment level, to be settled in the next revision or in the pre-analysis amendment. The administrative items remain on the checklist as pre-registration conditions. I would approve the methods for registration once those conditions are completed.
VERDICT: ACCEPT
