major revision

The protocol is not ready for registration. The node list, several outcome definitions, and the population rules would produce the wrong comparisons if they were applied as written.

Major

  1. Node identities and the dose-response scale are wrong (nodes.csv; Additional models). The note on SEMA_PO_25MG_QD and SEMA_PO_50MG_QD calls these the original oral formulation and defers nodes for the reformulated 1.5, 4, and 9 mg tablets. The 25 mg and 50 mg doses are the higher-dose oral program. The reformulated Rybelsus tablets are a different product, already marketed, and bioequivalent to 3, 7, and 14 mg, not to 25 or 50 mg. Published randomized maintenance doses are missing: retatrutide 0.5 mg (diabetes phase 2) and 1 mg (obesity phase 2); orforglipron 45 mg (phase 2); beinaglutide 0.1 mg three times daily; survodutide 0.6 mg (obesity phase 2). Agents already named in concepts.yaml or node_classes.csv have no node, including cotadutide (100, 200, and 300 micrograms once daily, not weekly), pemvidutide, amycretin, maridebart cafraglutide (monthly), danuglipron, VK2735, ecnoglutide, HRS9531, and HRS-7535. "Dose response within molecule" would place oral and subcutaneous semaglutide on one milligram axis. Oral 14 mg is not subcutaneous 14 mg. Replacement: Remove the reformulation note from the 25 mg and 50 mg rows. Add nodes for reformulated oral semaglutide 1.5, 4, and 9 mg, and allow a sensitivity analysis that pools only 1.5 with 3, 4 with 7, and 9 with 14. Add the missing doses and one node for every agent already named in the concept file. Fit Emax models within one molecule, one route, and one formulation.

  2. Several outcomes cannot be pooled as coded, and the kidney composite is two different endpoints (outcomes.csv O13, O22, O24, O50, O100, O110). O13 is a mean difference of either BMI standard-deviation score or raw BMI. Those units cannot share a mean difference, and the threshold of 0.2 SDS does not apply to kg/m2. O22 accepts a 40 percent or a 50 percent eGFR decline and allows cardiovascular death inside the composite. O50 requires 50 percent and kidney death. A 40 percent composite and a composite that includes cardiovascular death (the FLOW primary endpoint) are not the same hazard ratio. O24 and O100 mix a mean difference and an SMD while the threshold is in one instrument's units. O110 is continuous_or_binary and MD_or_OR, direction lower_better, threshold blank. Abstinence is higher better; heavy-drinking days are lower better. P14 requires one network per substance, which this row cannot support. Replacement: Split O13 into BMI SDS (mean difference, threshold 0.2) and, only when SDS is absent, percent change in BMI. Define primary O22 as kidney failure, sustained eGFR decline of at least 50 percent, or kidney death. Extract a 40 percent composite, or any composite that adds cardiovascular death, as O22b and do not pool it with O22. For O24, fix direction per instrument, use the SF-36 physical-component mean difference with a threshold of 3 points when that instrument is shared, and use an SMD of 0.2 only when instruments differ. For O100, analyze CDR-SB as a mean difference with threshold 1 point, and keep ADAS-Cog separate. Replace O110 with one row per substance and endpoint, each with its own measure, direction, and threshold.

  3. Population assignment contradicts itself, and the diabetes strata omit common trials (Participants; populations.csv P01). One bullet requires the whole sample to meet the definition. The next admits a trial at 80 percent or by "a subgroup result," which could be post-randomization. S0, S1, and S2 omit prandial insulin, premixed insulin, and mixed background, so cardiovascular outcome trials and many glycemic trials match no stratum. Replacement: Assign a trial when at least 80 percent of participants meet the definition at baseline, or when a baseline-defined subgroup is reported. A trial may enter more than one population and enters each network once. Add S3, prandial or premixed insulin with or without other drugs, and S4, mixed background (no stratum reaches 80 percent). Build glycemic and weight networks in S0 through S3 only.

  4. The shared time windows drop the primary outcome of some populations (O01, O10; P04, P05, P12, P15). O10 allows weeks 52 to 80. PCOS trials and antipsychotic weight-gain trials are commonly 12 to 26 weeks, so they never enter the outcome those populations list as primary. O01 starts at week 20, so many type 1 diabetes adjunct trials and some adolescent trials, often 12 to 16 weeks, contribute no HbA1c result. Replacement: For P02 and P03, percent weight change uses the prespecified primary time if it falls between 52 and 104 weeks; otherwise the closest time in that range. Add a separate weight outcome for P12 and P15: target 24 weeks, window 12 to 52. For P04 and P05, set the HbA1c window to 12 to 56 weeks, target 26.

  5. Harms are odds ratios over unstated follow-up, and some thresholds cannot be met (O04, O25 through O29, O32, O34, O36; Effect measures). The text promises rate ratios, but no row uses them. A participant-level odds ratio is not comparable across trials of different length. Hypoglycemia, gastrointestinal events, and pancreatitis are recurrent. Thresholds of 1 per 1,000 for thyroid neoplasms, NAION, and aspiration are larger than plausible absolute risks, so the imprecision rule cannot find an important harm. Replacement: Use a rate ratio with person-time as the primary effect for O04, O27, O28, and O29. Keep odds ratios for O25 and O26 only inside a shared window or with an exposure offset. Set the rare-event threshold to 0.1 per 1,000 at the horizon in item 7. If a harm network has fewer than 20 events, report arm counts and do not fit the logit model. Do not use a continuity correction.

  6. The PubMed renderer breaks class phrases, and the concept file merges two drugs (R/search_strategy.R, quote_term, pubmed_query; concepts.yaml key hrs9531). PubMed does not truncate inside quotes. Hyphenated terms are quoted with the asterisk left in place, so "GLP-1 receptor agonist*"[tiab] searches for a literal asterisk. The hrs9531 entry includes HRS-7535. HRS9531 is a GIP and GLP-1 dual agonist. HRS-7535 is an oral small-molecule GLP-1 receptor agonist with its own phase 2 doses (30, 60, 120, and 180 mg in obesity; 15 to 90 mg in diabetes). The CagriSema entry also searches bare cagrilintide, an amylin analogue with no GLP-1 activity. Replacement: Expand truncated phrases to explicit singular and plural words before quoting, and put no asterisk inside a PubMed quote. Give HRS9531 and HRS-7535 separate agents and nodes. Search "CagriSema" and "cagrilintide and semaglutide," not cagrilintide alone.

  7. Absolute effects, the reference node, and the time horizon are undefined (Effect measures; Confidence in cumulative evidence). "Median control group risk" does not name the node. Hazard ratios have no horizon, so a threshold of 5 or 10 per 1,000 is not an effect size. The minimally contextualized GRADE rule needs one reference. Placebo and standard care are separate nodes, and some networks will lack placebo. Replacement: Declare the reference before fitting: placebo if connected, otherwise standard care, otherwise the most frequent active node. Take the absolute risk from that node's median risk. For hazard ratios, use a 3-year horizon for O14, O20, O21, O22, O23, O40, and O50. Store the reference rule and horizon in outcomes.csv.

  8. Primary priors do not match the scale or the comparison mix (Bayesian network meta-analysis, Priors). μ ~ N(0, 100^2) on the logit scale is effectively flat and samples poorly when events are sparse. d ~ N(0, 10^2) on every log relative effect does not regularize rare harms. The Turner prior is restricted to pharmacological versus placebo or control comparisons and to log odds ratios. Much of each network is active controlled, and Turner and colleagues provide separate predictive distributions by comparison type. Nonrandomized syntheses are told to reuse that trial prior. Replacement: Use identity-link baselines N(0, (5 s_y)^2), and logit baselines N(0, 2^2). Use N(0, 2^2) for log odds and log hazard on efficacy outcomes, and N(0, 1^2) when the median reference risk is below 5 per 1,000. If more than half of the direct comparisons are active controlled, use the active-versus-active Turner distribution; otherwise use the placebo-controlled one. Record the choice before results are extracted. For nonrandomized models, use a half-normal prior on τ with scale 1 on the log relative-effect scale.

  9. The nonrandomized harms analysis cites a column that is not in the table (harms synthesis; outcomes.csv). No field marks which harms are eligible, and no rule states the minimum design or how partial database overlap is resolved beyond a general hierarchy. Replacement: Add nrs_synthesis = yes for O20, O21, O23, O25, O26, O28, O29, O32, O33, O34, O35, and O36 only. Pool adjusted estimates, separately by design and by active versus non-user comparator. Prefer one new-user active-comparator estimate per data source, then the longest follow-up, then the largest sample. Never pool these estimates with trials in the primary analysis.

  10. Heart failure and chronic kidney disease are each defined as both one network and two (populations.csv P07, P08; effect_modifiers.csv). Preserved and reduced ejection fraction are split only "when both have connected evidence," and mildly reduced ejection fraction is unassigned. CKD with and without diabetes is one network, but diabetes is not a prespecified modifier for P08. Replacement: Primary networks are HFpEF (ejection fraction at least 50 percent), HFmrEF (41 to 49 percent), and HFrEF (40 percent or lower), plus diabetic kidney disease and CKD without diabetes. Pooled heart failure and pooled CKD are sensitivity analyses.

  11. CINeMA cannot be applied as written, and the thresholds are not protected from the results (Confidence in cumulative evidence). Contributions come from frequentist netmeta and intervals from the Bayesian model. Those are different estimates when zero cells make the inverse-variance and binomial models diverge. No number is given for how much high-risk-of-bias weight, or how large a node split, forces a downgrade. Thresholds can still be set after effects are seen. Replacement: Apply the six CINeMA domains separately to the frequentist estimates and to the Bayesian estimates. Where the category differs, keep the lower. Write numeric downgrade rules into the protocol. Lock every threshold in outcomes.csv before any outcome result is extracted.

  12. Eligible trial types and conflicting sources have no decision rule (Study designs; Data collection; Data synthesis). Cluster trials have no design effect. Crossover trials have no paired likelihood, despite the eligibility text. The source hierarchy asks at once for the most complete follow-up and the prespecified analysis, so a long post hoc paper and a regulatory review of the primary estimand both qualify. Completers analyses are not mentioned. Replacement: Multiply cluster-trial contrast variances by 1 + (mean cluster size minus 1) times ICC, using the trial ICC or 0.05 if it is missing, and flag the imputation. Enter a crossover paired contrast only when its standard error is reported; otherwise enter the first period. Prefer the prespecified treatment-policy result with the longest follow-up of that estimand, then the regulatory review of that analysis, then the registry, then the journal paper. Flag trial-product, completers, and per-protocol results and exclude them in the prespecified sensitivity analysis.

  13. Items required before registration are still open (Administrative information; Information sources; Appendix A; Search strategy). Reviewers, funding, and competing interests are unfinished, and the protocol itself forbids screening until two people are named. Embase and CENTRAL are undecided. Europe PMC does not replace Embase. Appendix A has no rendered strategy. PRESS 2015 has not been done. A living search tied to a closed name list has no source for new INNs. Replacement: Before registration, name at least two reviewers and the information specialist, state the funder and a 36-month competing-interest declaration, either search Embase and CENTRAL or record that they will not be searched, paste the version 1.0.0 queries into Appendix A, finish PRESS, and check proposed WHO INN lists every quarter, logging every added name as an amendment.

Minor

  1. node_classes.csv row GLP1_SA says beinaglutide has a half-life of hours. It is native GLP-1, with a half-life of minutes. Replacement: "Exenatide twice daily and lixisenatide, half-life about 2 to 4 hours; beinaglutide, half-life of minutes."

  2. Comparator nodes do not follow the molecule rule (nodes.csv INS_PRANDIAL, INS_PREMIX; class GLINIDE). Basal insulins are split by molecule. Prandial and premixed insulins are single nodes. Repaglinide, a common comparator in the Chinese trials, has a class and no row. Replacement: Separate insulin aspart, lispro, glulisine, and regular human insulin. Add repaglinide, nateglinide, and one OTHER_ACTIVE node so an unlisted comparator can be coded before the next amendment.

  3. Default frequentist binary analysis adds 0.5 to zero cells (Frequentist network meta-analysis). In netmeta, pairwise uses incr of 0.5 for odds ratios when a cell is zero. The protocol forbids continuity corrections and treats Mantel-Haenszel as extra. Replacement: When any arm has zero events, the primary frequentist model is netmetabin with method Mantel-Haenszel and no continuity correction. Use inverse-variance netmeta only when every arm has events, with incr set to 0.

  4. The nonrandomized variance method is misnamed (harms synthesis). "Hartung, Knapp, Sidik and Jonkman adjustment" joins two different procedures. Replacement: Estimate τ² by REML, use Sidik-Jonkman if REML is unstable, and apply the Hartung-Knapp adjustment to the interval.

  5. EM02 puts baseline BMI in kg/m2 on every population, including children. Replacement: For P03, the modifier is BMI standard-deviation score.

  6. Two provisional thresholds will not sort important from trivial effects (O05, O10, O12). A 2.5 percentage-point weight difference is below the usual 5 percent criterion. A 2 mmHg systolic difference will be crossed by almost every active comparison. Replacement: Use 5 percentage points for O10 and O12 and 5 mmHg for O05, unless patient partners lock different values before extraction.

  7. WOMAC and eGFR slope need a unit convention (O51, O80). A 10-point WOMAC threshold fits a 0 to 100 scale, not the 0 to 20 or 0 to 50 versions. Slope is higher-better only when loss is negative. Replacement: Rescale WOMAC pain to 0 to 100 before pooling. Define eGFR slope as mL/min/1.73 m2 per year, with decline negative.

  8. Two populations lack the endpoint their trials were built around (P04, P05). Adolescent type 2 diabetes has no hypoglycemia outcome. Type 1 diabetes adjunct trials routinely report total daily insulin dose. Replacement: Add O04 to P04. Add change in total daily insulin dose (units per kg per day), mean difference, lower better, window 12 to 56 weeks, threshold 0.1 units/kg/day, for P05.

  9. MACE has no mapping rule (O21). A four-point primary endpoint that adds unstable angina, as in ELIXA, is not three-point MACE. Replacement: Prefer adjudicated three-point MACE. Extract a four-point composite as a separate outcome and do not pool it with O21.

  10. Network meta-regression will overfit, and component models do not fit fixed-ratio products (Network meta-regression; Additional models). Fifteen covariates, including funding and blinding, may be fit as class interactions once ten trials exist. IDegLira and iGlarLixi do not deliver independent component doses. Replacement: Fit one centered clinical covariate at a time from EM01 to EM11, only when at least ten trials observe it, with the interaction prior N(0, 1^2) on the standardized covariate. Keep funding, blinding, year, and estimand in bias sensitivities. If the covariate is missing for more than 20 percent of trials, downgrade for intransitivity instead of regressing. In component models, code basal insulin and the GLP-1 molecule as present or absent. Keep fixed-ratio nodes out of the Emax model.

  11. OpenAlex is not restricted to title and abstract, and screening calibration is raw agreement (openalex_queries; Selection process). The function returns a bare Boolean string. Agreement above 90 percent is easy when almost every record is excluded. Replacement: Store the fielded OpenAlex query that is actually sent. On a random calibration set of 300 records, require agreement above 90 percent and Cohen's kappa of at least 0.70.

VERDICT: MAJOR REVISION
