Peer review: LivingGLP1 protocol, version 0.1.0
I verified the software claims against the installed packages (multinma 0.9.1 and netmeta 3.7.0): class effects, node splitting, the complementary log log link with a log follow-up offset, mixed arm level and contrast level data, REML, and the Mantel-Haenszel network method all run as described, and E-utilities honors truncation inside the quoted phrases the renderer sends. The problems below are in the rules and the tables, not the tools.
Major
outcomes.csv O10 (percent body weight change) carries target week 68 and window 52 to 80 for P12 and P15. Published GLP-1 trials in polycystic ovary syndrome and in antipsychotic associated weight gain run 12 to 36 weeks, so both networks are empty as written and these populations lose their only weight outcome. Replacement: make the target and window population specific (P02, P03: 68, 52 to 80; P12, P15: 26, 12 to 40), as extra columns or as rows O10A and O10B.
outcomes.csv O110 cannot be executed: type
continuous_or_binary, measureMD_or_OR, directionlower_better(wrong for abstinence) and an empty threshold. Replacement: one row per substance and construct, for example O110A heavy drinking days (continuous, MD, lower_better, 26 weeks, 12 to 40, threshold 2 days per month) and O110B abstinence (binary, OR, higher_better, threshold 50 per 1000), with tobacco, opioid, stimulant and cannabis rows built the same way.outcomes.csv O11 ("analyzed per threshold") and O27 ("each analyzed separately") pack four or five syntheses under one outcome_id, which the data package uses as a key. Replacement: rows O11A to O11E (5, 10, 15, 20, 25 percent) and O27A to O27D (nausea, vomiting, diarrhea, constipation).
outcomes.csv O32, O34 and O36 set 1 per 1000 as the threshold for thyroid neoplasms, NAION and aspiration. Control group risks for these events over a trial are well below 1 per 1000, so no estimate could cross the threshold and every result would be labeled "little or no difference" whatever the odds ratio. Replacement: for harms with median control risk below 2 per 1000, define the threshold on the relative scale (OR 1.25), present absolute effects per 10,000, and record the rule in the outcomes section.
The nonrandomized harms section refers to harms "marked for nonrandomized evidence" in outcomes.csv; no such column exists, so the harms synthesis has no defined outcome set. Replacement: add column
nrs_synthesis(yes or no), set it for O14, O28, O29 and O32 to O36, and add long latency harms with no trial counterpart (pancreatic, thyroid and overall cancer; gastroparesis and bowel obstruction; pregnancy outcomes; suicide) with definitions and thresholds.populations.csv P01 with outcomes.csv O02: the only weight outcome in type 2 diabetes is kilogram change at 26 weeks (window 20 to 56). Weight management trials in diabetes (STEP 2 at 68 weeks, SURMOUNT-2 at 72 weeks) therefore contribute nothing at their primary time point, while their obesity counterparts do. Replacement: add P01 to the population list of O10 and O11, and state in Participants that weight management trials in diabetes are analyzed in the stratum of their background therapy.
populations.csv P01 strata: S2 is "basal insulin with or without other drugs", so trials on prandial, premixed or basal bolus insulin (AWARD-4; liraglutide added to multiple daily injections) have no stratum and cannot enter the glycemic networks. Replacement: "S2: any insulin regimen, with basal only versus other regimens as a covariate", and state that the 80 percent rule in Participants also governs stratum assignment.
Networks and nodes says shared background does not define a node, but Eligibility does not exclude trials in which the GLP-1 containing regimen is itself the shared background and the randomized contrast is exercise, diet or a non GLP-1 drug. Such trials map every arm to one node and produce a self loop. Replacement, under Comparators: "A trial enters a network only when at least two arms map to different nodes of that network; trials whose randomized contrast does not change the GLP-1 containing regimen are inventoried only."
Networks and nodes has no rule for flexible dose or maximum tolerated dose arms (PIONEER 7, SURMOUNT-5), for free combinations randomized as a unit (exenatide plus dapagliflozin in DURATION-8), or for the node a dose randomized comparator forms (the "unless a trial randomizes doses" clause ends without a consequence). Replacement: flexible and maximum tolerated dose arms form a node
<agent>_FLEXper route and frequency, with a sensitivity analysis assigning them to the highest permitted dose; a free combination forms a node named by both components; a comparator with randomized doses keeps dose specific nodes in that network and pooled doses elsewhere.populations.csv P07: "HFpEF and HFrEF analyzed separately when both have connected evidence" makes a transitivity decision depend on the data and pools phenotypes with different prognosis and drug response whenever one is sparse. Replacement: P07A (HFpEF, ejection fraction 40 percent or more) and P07B (HFrEF) as fixed populations, each subject to the connectivity rule and never pooled.
Priors: heterogeneity priors are given for binary (Turner) and continuous (half normal) outcomes only. Time to event and recurrent event outcomes, which include every mortality, cardiovascular and kidney network, have none, and the Turner comparison type is not fixed for networks that mix placebo and active comparators. Replacement: "Log hazard and log rate scales use the Turner distribution for the matching outcome type as an approximation; the comparison type is pharmacological versus placebo or control for all networks; both choices are fixed in analysis/priors.csv."
Effect measures with outcomes.csv O25 to O36: binary harms use the odds ratio at longest follow-up, so 12 week and 5 year trials are pooled on a scale in which the odds grow with exposure time. The complementary log log model with a log follow-up offset that the protocol already specifies for counts (it fits in multinma) should be primary here. Replacement: "Binary harms with window 'any' are modeled on the log hazard scale (binomial likelihood, complementary log log link, offset log follow-up time); the odds ratio model is a sensitivity analysis."
Information sources and Appendix A: PRISMA-P item 10 requires the full draft strategy for at least one database in the protocol, and Appendix A is empty although a rendered run exists in search/runs/2026-10-12. Embase and CENTRAL are "TO BE COMPLETED", which MECIR C24 does not allow for a review of randomized trials, and no Chinese language database is searched although some beinaglutide, PEG-loxenatide, supaglutide, bofanglutide and HRS9531 trials appear only in Chinese journals. Replacement: paste the rendered PubMed and ClinicalTrials.gov strategies now; commit to CENTRAL and state the Embase decision; add CNKI and Wanfang with the agent names in Chinese, or justify their omission.
nodes.csv is already behind the evidence: cotadutide, efinopegdutide, pemvidutide, danuglipron, ecnoglutide, maridebart cafraglutide and amycretin have published randomized trials but no node; orforglipron 45 mg, survodutide 0.6 mg, tirzepatide 1 mg and retatrutide 1 mg were used in published phase 2 trials; RETA_SC_9MG_QW matches no published dose (phase 2 used 0.5, 1, 4, 8 and 12 mg) and should be verified against the TRIUMPH registrations or removed. "New regimens are added by amendment" then guarantees amendments at baseline. Replacement: complete the node list from the registry harvest before registration and keep the amendment rule for regimens that are new after it.
Data collection: one extractor plus a verifier does not meet the standard for outcome data in a network meta-analysis (MECIR C47 requires independent duplicate extraction). Replacement: "Outcome data used in any synthesis are extracted independently by two reviewers; study and arm characteristics are extracted by one and verified by a second."
Minor
Outcomes and time points says critical outcomes are listed in populations.csv, but that file has
primary_outcome_idsand criticality lives in outcomes.csv. State: "Critical outcomes for a population are those with importance critical whose population_ids include it; primary outcomes are the subset named in populations.csv."outcomes.csv O22 and O50 mix 40 and 50 percent eGFR decline and composites with and without cardiovascular death. Add: "the trial's prespecified kidney composite is used, its components are recorded, and a sensitivity analysis restricts to composites without cardiovascular death."
O04: trials before the 2017 ADA and EASD classification report "minor", "confirmed below 3.1 mmol/L" or "documented symptomatic" hypoglycemia. Add a mapping rule (confirmed below 3.1 mmol/L maps to level 2; severe or major episodes requiring assistance map to level 3).
populations.csv P14 uses one code for five substance networks; assign P14A to P14E so the network key is unambiguous. The P99 note repeats "connectivity rule"; nodes.csv copies the dose pooling note onto PLACEBO, STANDARD_CARE and LIFESTYLE, where it has no meaning.
effect_modifiers.csv has no population specific modifier for P07 to P15 (ejection fraction, baseline fibrosis stage, baseline AHI and CPAP use, amyloid status, antipsychotic type), and the EM11 "category" is undefined. Add rows and define EM11 as none, standard advice, or intensive behavioral therapy.
Selection process: "agreement on inclusion exceeds 90 percent" names no statistic. Use Cohen's kappa of at least 0.8 on the calibration set, with percent agreement reported.
Data collection hierarchy: "most complete follow-up and the prespecified analysis" can conflict. Order it: prespecified estimand first, then completeness of follow-up, then source type.
Frequentist network meta-analysis: name the contribution matrix algorithm (netmeta::netcontrib, shortest path or random walk), because the two give different CINeMA inputs.
Network meta-regression: within trial subgroup results cannot feed multilevel network meta-regression, which needs individual participant data in at least one trial; say they are extracted for aggregate subgroup meta-regression.
Confidence in cumulative evidence rates critical outcomes only; summary of findings tables need important outcomes too. Rate both.
Rationale overstates: heart failure and osteoarthritis pain are investigated, not approved, uses. Write "approved for ... and studied in ...".
concepts.yaml and Search strategy: the claim that the receptor and hormone MeSH headings add no drug studies is untested; report in the run log the count of records retrieved by "Glucagon-Like Peptide-1 Receptor/agonists"[Mesh] NOT (the rest of the strategy). Add terms "dual agonist*", "triple agonist*", "tri-agonist*", "polyagonist*", "incretin agonist*", the cagrilintide codes (AM833, "NNC0174-0833"), amycretin (NN9487), noiiglutide (SHR20004), PB-119, efocipegtrutide (HM15211), GZR18, CT-388 and HS-20094.
Items 1, 2, 5 and 13 make the protocol non-executable as written for named populations and leave a PRISMA-P item unmet; items 6 to 12 are analysis decisions that must be settled before any data are seen.
VERDICT: MAJOR REVISION
